Please note, the following documentation must be presented in original format to the school office for verification:
- A Certificate of Catholic Practice (if relevant).
- Applicants from other Christian denominations and other faiths may provide either a certificate of baptism or a letter from their minister or religious leader, confirming membership of that faith community.
- You must complete the Local Authority’s application form online (or on paper) by the closing date. If you do not do this, you will not be offered a place.

- Two proofs of address in the last 3 months to include Council Tax Bill/Rent Agreement / Mortgage Statement / Child Benefit Letter / GP Letter

- Does your child have any special educational needs? (Please include evidence where applicable)
